We are now registering New patients

All new patients applying to join the practice will be asked to complete a simple application form (available from reception) and, if accepted by the Practice, you will be given a joining pack which includes:

​New Patient Registration Information and GuidanceNew Patient Registration Form – to complete for registrationNew Patient Information Questionnaire – to complete for registration

Once we have received the Registration Form and Information Questionnaire the actual registration will be completed.  You will be informed of your “Named GP” at this stage. The new patient information questionnaire should be completed in full as this enables us to identify your health status and any special requirements you may have to help you access our services. In addition, you are invited/encouraged to make an appointment for a health check within 1 month of joining the Practice. The health check gives us the opportunity to update your health status and enables us to provide health services and lifestyle advice appropriate to your needs.

We are also taking on asylum seeker patients please feel free to enquire with our surgery.
